How Michigan’s public mental health system works

Most people only encounter this system during an emergency, and the vocabulary gets in the way. Here is the short version.

Your CMHSP is the front door

A Community Mental Health Services Program is the public mental health agency for your county. There are 46 of them covering Michigan’s 83 counties — some serve one county, others serve six. They handle crisis response, and they are the entry point for ongoing public mental health services.

PIHPs manage the money, not your call

The 46 agencies are grouped into 10 regional entities called Prepaid Inpatient Health Plans, which administer Medicaid behavioral health funding for their region. You almost never need to contact a PIHP directly. It is listed above only because you will see the name on paperwork.

Crisis line, access line, and customer service are three different numbers

This trips up more people than anything else. The crisis line is answered around the clock for emergencies. The access line is for requesting ongoing services and usually runs on business hours. Customer service handles benefits questions, grievances, and recipient rights — it is not a route to care. Each is listed separately above so you do not lose time on the wrong one.

Mobile crisis teams come to you

Many agencies can send trained staff to a home, school, or other location rather than requiring you to travel during a crisis. Some publish a direct dispatch number; others route the request through the main crisis line. Both are noted per county.

Walk-in crisis centers are an alternative to the ER

A growing number of Michigan agencies operate crisis stabilization units, behavioral health urgent care, or walk-in crisis clinics. For a mental health crisis without a medical emergency, these are usually a better fit than an emergency department — shorter waits, staff trained for exactly this, and a direct path into follow-up care.

Common questions

Should I call 988 or my county crisis line?

Both work and both are free. 988 is the national Suicide & Crisis Lifeline — it always answers, and it is the right call if you are not sure who to contact. Your county line connects you directly to the agency that can send a mobile team, arrange a face-to-face screening, or admit someone locally, so it is often faster for anything involving in-person help.

Do I need insurance to call?

No. Community mental health agencies in Michigan provide crisis services regardless of insurance status, and calling costs nothing. Payment and coverage are worked out afterward, not before you get help. If you are worried about cost for ongoing services, ask when you call — agencies can explain sliding-scale and Medicaid options.

What happens when I call?

A trained worker answers and asks what is happening, who is involved, and whether anyone is in immediate danger. Depending on what they hear, they may talk with you on the phone, arrange a screening, send a mobile crisis team to you, or help arrange transport. You are not committing to anything by calling.

What is a CMHSP?

A Community Mental Health Services Program is the public mental health agency for a specific county or group of counties. Michigan has 46 of them covering all 83 counties, grouped under 10 regional entities called PIHPs that manage Medicaid behavioral health funding. Your CMHSP is the front door to public mental health services where you live.

Can someone come to me instead?

Often, yes. Many Michigan agencies operate mobile crisis teams that travel to a home, school, or other location. Some publish a direct number, which is listed above where one exists. Where no separate number is published, ask for the mobile crisis team when you call the main crisis line.

Should I go to the emergency room?

Call 911 or go to an emergency room for a medical emergency, an overdose, or a life-threatening injury. For a mental health crisis without a medical emergency, calling the crisis line first is usually faster and more appropriate — many agencies operate walk-in crisis centers or urgent care sites designed for exactly this, and those are listed above where they exist.

How this list was built

The agency list and regional structure come from the Michigan Department of Health and Human Services and the Community Mental Health Association of Michigan. Every phone number was then read from the serving agency’s own website or confirmed by telephone; each county entry shows which, and the date it was last checked.

  • All 83 counties are covered, with one serving agency each.
  • Where an agency publishes both a local and a toll-free crisis number, both are listed. They reach the same place.
  • Where an agency offers mobile crisis response but publishes no separate number, that is stated rather than guessed.
  • 988 is listed as the statewide entry point because MDHHS identifies MiCAL as Michigan’s 988 call center rather than a separate hotline.
  • Numbers change. If something here is wrong or out of date, please tell us and it will be corrected.
